2025-26 New Viking Medical Packet

All athletes new to Bethany Lutheran must complete the following:

1. Health History Form (in link above)
2. Physical Exam
3. Clearance Form
4. Sickle Cell Trait Verification
5. NCAA Medical Exceptions Reporting Form - if taking NCAA banned substance(s)
6. Healthy Roster Online Paperwork - complete all information needed (see below for directions)

As of fall 2012, student athletes using a banned stimulant drug to treat Attention Deficit Hyperactivity Disorder (ADHD) or Attention Deficit Disorder (ADD), need to fill out the NCAA Medical Exceptions Reporting Form. Your physician must sign this form and attach supporting documentation.
